An off the rack suit is made to fit as many men as possible. Alterations are made where possible to try and achieve a better fit but compared to a made-to-measure suit these alterations are not even close to being able to achieve a good fit.

First there is the Athletic man who has a much smaller waist than his chest. An off the rack suit is made with a 6' drop which means that if a man is a 42 regular in the chest than the pants that come with that coat are a 36' waist. Problem is that someone with an athletic build will have more like a 34' or a 32' waist. A good tailor can take the waist on the pants in a couple inches but after that the alterations start to affect the fit of the garment in a negative way.

Not only will the tailor need to take in the pant waist, he will need to take in the coat body as well to give a more tailored look. Same problem here, he can only take the coat in a couple inched before ruining the integrity of the garment.
