Please send me an application for my daughter's roommate who is unemployed and needs only one course to complete and money for books and cost to take the certiffication test to become a Sleep Apnea Lab technician, a job she held for three years in Florida. She needs to be certified to work in Colorado. She is unable to find a job in this field without being certified. My daughter is assisting by letting her live with her until she can get back on her feet and be gainffully employed. thank you for your assistance. I am a School Nurse and you have helped me in the past with student needs.

Hello, I am seeking to get financial assistance to buy a continuous portable Oxygen concentrator, please send the application. Member is a 64 year old man who is currently a home bound. He is on Oxygen continuously and he can only leave his home for doctors appointments, member would like to go out with wife and family and to be active in his community but he cannot leave his home for long periodof time because the portable Oxygen tanks covered by his insurance can only last him 1-2 hours .
